It's here! As of June 6, 2024, Licensed Midwives in WA State can apply for up to three License Extensions. We have compiled everything you need to know to apply for one or all extensions.

​When you are ready to apply, go to the DOH Licensing Website, click Apply on Paper, and select one or all of the licenses you want to apply for.
